From the Cornerstone catalogue: "the official mintage shows only six sets struck, with only one original set containing a specimen strike of the five cent coin." This coin is the lone example from an original full 1938 specimen set. It has since been regraded, as Cook cracked out all of his coins and put them in 2x2s, but its previous(now obsolete) cert number is consecutive to the original set from which it originates. Luckily this coin appears to have gone unharmed by Cook, as were many of his high quality nickel coins, some of which even upgraded from SP68 to SP69 after his collection was re-certified.

Belzberg. Quite possibly from a specimen set originating from Maurice Lafortune, as Belzberg came into possession of some superb gem 1948 dollars in both MS and SP from Lafortune. The Lafortune origins are unverified currently but very possible.
Lafortune-Pittman, ICCS SP-67 Pittman lot 2207 the second coin from a two-piece lot consisting of a 1948 and 1949 specimen nickel duo.
Lafortune-Pittman, Heavy Cameo. Pittman lot 2509, originating from an original set acquired by JJP directly from mint employee Maurice Lafortune

Formerly an old NGC SP65, also cross-graded ICCS SP-65. Originating from a full NGC graded 1944 specimen set that was graded SP65 and up.
Norweb lot 271. After digesting his large acquisition of 1940s-50s specimen sets from Mint employee Maurice LaFortune in 1952, Pittman appears to have sold a decent amount of them to the Norwebs in 1954 to recover some of his money from duplicates. An identically toned quarter is recorded in the Norweb sale as coming from Pittman, it is safe to assume that this dime has the same origins.
